According to Mojola, preliminary investigations revealed that there was zero employee training, very poor housekeeping, and no health and safety policy or safety management system.

Mojola also said there was unavailability of personal protective equipment for workers and poor welfare for staff.

“Furthermore, there was poor emergency response following the accident. However, further investigation is still ongoing as of press time,” the Director-General said.
